Baltimore in 1971: John Kelly, a former highly decorated and mentally battered veteran of the Vietnam War , who lost his pregnant wife in a car accident six months earlier, accidentally meets and falls in love with the young Pamela Madden. He takes her to his own island, Battery Island , where he lives. On the way, the two get to know the doctor couple Sam and Sarah Rosen. When they discover tranquilizers in Pam's luggage, their dark past comes to light.

The drug addict Pam has escaped a brutal pimp and drug smuggling ring . She was brutally abused by them and made addicted to drugs.

John helps her back to life and takes care of her. He gets support from Sam and Sarah Rosen. But John Kelly is spared nothing. Pam is discovered by her ex-pimp and his accomplices . After a chase, Kelly makes a fatal mistake. Pam is cruelly killed and Kelly is badly injured.

In the hospital, he meets the nurse Sandy O'Toole. Physically and mentally battered and desperate, Kelly embarks on a brutal campaign of revenge against the gangsters and corrupt police officers. This mafia-like organization hides heroin in the corpses of American soldiers and smuggles it into the USA. In addition, Kelly has to fight on a second front.

He is given the job of freeing American officers from prison in a prison camp in Vietnam. Since nobody knows the area in question as well as he does, he cannot avoid the secret mission. From now on he lives in a constant conflict between his private campaign of revenge and his military mission. Unfortunately, betrayal and corruption prevail here too , the project is betrayed by an insider and fails.

In Baltimore, Kelly continues his campaign of revenge. Thanks to his skill and experience as a former Navy Seal , he can kill Pam's murderer and the gang members involved. He also manages to free Doris, Pam's friend. He asks Sandy O'Toole to take care of her and goes on the mission in Vietnam. Weeks later, Doris is also tracked down by the gangsters and killed.

While Kelly finishes his campaign of revenge, the police track him down. During a chase with a police boat, Kelly spectacularly fakes his death by colliding with a cargo ship. With the help of a compressed air cylinder , he remains undetected because three US admirals enable him to escape with a Navy sailing ship .

During his adventures, Kelly met some people who played important roles in other episodes of the Jack Ryan novels: Admiral Greer and Robert "Bob" Ritter, both later senior CIA employees, "Portagee" Oreza (Coast Guard), Admiral Painter and the treacherous Senate employee Henderson, who, with Kelly's help, is "turned over" as a KGB agent.

John Kelly gets a new identity and begins a new life as John Clark together with Sandy O'Toole in Virginia and joins the CIA .
