Registration form is the name of the form used in the disaster control telecommunications service for the transmission of reports, messages and instructions. This is a form with three carbon copy forms on which incoming and outgoing messages are noted, their route of transport is noted and the response noted. With the help of the registration form, instructions from operations managers or section heads , or reports from emergency services to the operations / section command, can then be precisely reconstructed.
