The film begins as reporter Angela Vidal (Jennifer Carpenter) is taping a news segment outside a Los Angeles fire station with her cameraman Scott Percival (Steve Harris). During a tour of the station, Angela meets firefighters Fletcher (Jonathan Schaech) and Jake (Jay Hernandez), whom Angela and Scott will be following for the evening.

As Angela begins to grow bored with the lack of action at the station, the alarm sounds. Angela, Scott, Fletcher, Jake, and two other firemen respond to the incident, described as a medical emergency at an apartment complex. Yuri (Rade Serbedzija), the building superintendent, called because of a woman screaming in an apartment.

Yuri, Angela, Scott, Fletcher, and Jake enter the building to find some tenants and two police officers, James (Andrew Fiscella) and Danny (Columbus Short), in the lobby. The firefighters, the reporters, James, and Yuri all go up to the apartment of Ms. Espinoza (Jeannie Epper), the source of the screaming. Espinoza is unresponsive to their knocks, so Jake breaks the door down. A dog runs out of the apartment as the group enters. Espinoza is found in the living room, struggling to breathe. She is startled by the light from Scott’s camera and appears to be covered in blood, foaming at the mouth, and growling. She bites James, but is subdued by the others. Fletcher remains with Espinoza as the others take James back to the lobby. The group tries to leave the building to get medical assistance, but they are locked in by the authorities and told not to leave. Suddenly, Fletcher’s body falls down the stairs. He is bleeding from a bite to the face. Lawrence (Greg Germann), is a veterinarian, and tends to the wounded men.
* keep I've never seen an AfD on a character where that character pulls up 700 news articles (click on the link above). Looking at the sources, most of them are about the movie itself, not the character per se. But for example is certainly better than a movie review (it is a serious article asking what exactly the movie (and the main character) tapped into to be so popular. The actor was so popular in the role that he's looking at running for parliament () and when he's discussed, is in terms of this hugely popular character . I'm going to go out on a very short limb and say this is notable without finding anything better. Hobit (talk) 02:33, 10 October 2008 (UTC)

Danny and Jake go upstairs to subdue Espinoza, and they order the reporters to stay downstairs. Angela and Scott sneak upstairs, and encounter a cleaning woman who falls dead in front of them. They find Danny, Jake, and Espinoza in the apartment. Danny shoots Espinoza as she charges them. They all return to the lobby, meeting Sadie (Dania Ramirez) on their way.

By this time, all exterior windows and doors are being sealed, and cell phones are being jammed. Anyone who attempts to leave is forced back at gunpoint. Jake believes they are being quarantined due to a biological or chemical emergency in the building. He goes door-to-door to round up all the tenants and bring them to the lobby. Jake, Angela, and Scott attempt to retrieve Elsie (Stacy Chbosky) from her apartment, but she is unresponsive. The group carries her down to the lobby with the others.

There is a textile shop attached to the building. Yuri opens the shop and the group places Fletcher, James, and Elsie inside. Yuri then tries to lead the group out of the building by another way, but that too is blocked. Back in the lobby, Angela begins interviewing tenants. One little girl, Briana (Joey King), is sick. Briana claims that she has a dog named Max who was sick and taken to an animal hospital. Lawrence states that the symptoms of those infected are similar to rabies; however, the infection takes effect much quicker.

Angela and Scott notice Bernard and Sadie sneaking away to the textile shop, looking for a TV to try to discover what is happening outside. Espinoza’s rabid dog appears and attacks Randy. The group then encounters Elsie. She attacks Angela and is killed by Scott. On TV the group hears the chief of police claim that everyone has been evacuated from the building, just before the power is shut off.

Back downstairs, Fletcher tries to attack everyone in the lobby, but Danny injects him with a sedative and then rounds everyone up in the textile shop. Danny states that CDC agents are coming in to give blood tests. Anyone who is not infected will be released. Lawrence observes that the only way to test for rabies is through a brain sample, not a blood test. Danny argues that they are not sure what they are dealing with. After a roll call, Yuri reveals that the penthouse apartment of the building is being rented by a man from Boston who is absent.

The CDC agents arrive and examine James and Fletcher in the locked textile shop. Lawrence, Danny and Jake accompany them. Angela and Scott observe through an adjacent room as an agent drills into Fletcher’s head for a brain sample. Fletcher bites the agent. Danny, Jake, and the other agent escape, leaving Lawrence locked in the room with the infected. After an attempted escape, the textile shop is locked and the surviving CDC agent is forced to explain the situation: a sick dog named Max was brought to a vet with an unknown illness, infecting other animals in the office. The dog was traced back to the apartment building.

Briana’s fever is called into question, but her mother, Kathy, claims that her daughter is not infected. Briana bites Kathy and runs away. The group follows the girl; Danny is attacked by Briana in Espinoza’s apartment. Jake attacks Espinoza, who appeared at the apartment door. The other infected individuals break out of the textile shop. Kathy is killed.

The survivors lock themselves in a room. The surviving CDC agent has been bitten and runs away to keep from infecting the others. He is locked in an adjacent room while the others try to figure out what to do. Sadie is also bitten but Bernard pleads with the others not to kill her. Bernard cuts through a window seal to call for help, but is killed by a sniper. Yuri reveals that they can escape through a basement passage into the sewers; however, the keys are in his apartment. He is then killed by the infected agent.

After a number of encounters with infected tenants, Angela and Scott discover a key ring in Yuri’s apartment. As the only survivors, Angela and Scott are chased into the penthouse apartment. It is revealed that the man from Boston, who was renting the penthouse, was a member of a doomsday cult who were cultivating an "Armageddon virus". A noise from the attic prompts Scott to investigate, and find an infected boy. The boy destroys the light on Scott’s camera, so he turns on the camera's night vision. Angela and Scott see a thin man emerge from another room. He walks past them initially but attacks Scott when Angela inadvertently makes a noise. Angela watches through the night vision of the camera as the infected man eats Scott. Angela is then attacked and drops the camera. The final shot is of Angela clawing her way back to the camera only to be dragged back into the darkness.
