On July 28, 1976, an Ilyushin Il-18 had an accident on ČSA flight 001 from Prague to Bratislava . Shortly after taking off, the machine crashed into Lake Zlaté Piesky due to an engine failure .
Flight history
Flight OK001 took off at 8:52 a.m. from Prague Airport with destination Bratislava. During the preparations for landing, the first misunderstandings between pilots and air traffic controllers arose. When the pilots reported the descent to 9,000 feet (2,750 meters), the air traffic controller understood and confirmed 8,000 feet (2,450 meters). However, the pilots took this as an instruction and sank to 8,000 feet. The air traffic controller suspected flight OK001 was 44 kilometers from the airport, but in fact it was only 29 kilometers. Because the plane was still flying too high at this point, the pilots had to make a steeper approach than usual. The pilots approached runway 22 at Bratislava Airport at a rate of descent of 22 m / s instead of the normal 10 m / s . The Il-18 was too fast, which is why the pilots fully extended the flaps and set engines 2 and 3 to reverse thrust. Engine no. 3 failed and automatically switched to the sail position. However, the flight engineer accidentally switched engine 4 to the sail position. The pilots canceled the landing at a height of 50 meters and took off. The tower asked the pilots to make a left turn. But the Il-18 turned to the right and narrowly missed the airport tower. At a height of 40 meters, the flight engineer tried to restart engine number 4. In the meantime, the airspeed had dropped so much that the pilots lost control and the aircraft hit the Zlaté Piesky Lake with a pitch and roll of 60 ° each . Except for the stern, the aircraft sank completely in the water. Four passengers were rescued alive, one of whom later died in hospital. A total of 76 people died, making it the second worst aircraft accident in what is now Slovakia.

Cause of accident
After evaluating the flight recorder, the investigative commission determined the following causes for the crash:
- Use of thrust reverser below an altitude of 1000 meters
- Improper handling of the thrust reverser
- Falling below the minimum approach speed
- Accidental shutdown of engine # 4
- Not executed curve in the direction of the running motors
- The immediate cause, the attempt to start engine No. 4 at low altitude and speed.
