CURB-65

from Wikipedia, the free encyclopedia

CURB-65 is an acronym for a clinical assessment system for patients with pneumonia . The individual letters stand for the clinical findings or laboratory values in English, which are assessed for the score. Such scores are used to quickly assess the severity of a disease and the necessary intensity of therapy. Ultimately, the overall clinical assessment is always decisive. Especially in patients with additional illnesses, the score tends to underestimate the severity.

Score

The elements of the acronym stand for the following English terms:

  • C = Confusion: pneumonia- related abbreviated mental test score ≤ 8
  • U = urea (serum urea):> 7 mmol / L
  • R = Respiratory rate:> 30 / min
  • B = Blood pressure: Systolic below 90 mmHg or diastolic below 60 mmHg
  • 65 = patient is ≥ 65 years old

A maximum of 5 points are assigned, which are associated with different mortality rates and thus provide an indication of whether patients can be treated on an outpatient basis or should be treated as an inpatient or even intensive care.

  • Score 0: 0.7%
  • Score 1: 3.2%
  • Score 2: 13%
  • Score 3: 17%
  • Score 4: 41.5%
  • Score 5: 57%

Similar scores for risk stratification of community-acquired pneumonia (CAP) are the CURB index (without taking age into account ) and the CRB-65 index (without taking into account the urea in the serum).
