ISAR (medicine)

The ISAR test ( Identification of Seniors at Risk ) is a widely used tool in medicine for assessing the risk of seniors aged 75 and over at hospital admission. It is part of the geriatric assessment. In particular, it should be clarified at the time of admission whether the patient needs additional help at discharge. The questionnaire should be completed by the staff together with the patient or their caregiver and contains six questions, each of which can be answered with yes or no:

1. Need for help Did you need regular help before the illness or injury that brought you to the clinic?
2. Acute change in the need for help Have you needed more help in the past 24 hours than before?
3. Hospitalization Have you been in hospital for one or more days in the last 6 months?
4. Sensory restriction Under normal circumstances, do you have significant vision problems that cannot be corrected with glasses?
5. Cognitive limitation Do you have serious memory problems?
6. Multimorbidity Are you taking six or more different medications per day?

From two points onwards, there is a high probability that a special geriatric action is required. The ISAR test is recommended by the German Society for Geriatrics , but the validity is assessed differently in the literature.
