The foundation of the Church of St. Sebastian dates back to 1680 after an epidemic of plague raged in the city. An episcopal permit to build the church was granted on November 18, 1680. The church was built on the site of the old tavern from which the plague spread in 1679. This plague epidemic claimed more than 900 victims, which was more than 50% of the city's population at the time.

The construction of the chapel was delayed by several city fires and could not be completed until 1696. In 1711 the bell was cast. On August 1, 1720, the church was dedicated to St. Sebastian , the patron saint of plague sufferers. In 1785 there was extensive roofing work, with the roof being covered with new shingles. Around 1813 the chapel served as a hospital and warehouse. In 1838 the slater Drescher from Neisse put the little turret on the roof. Further renovation work took place between July and October 1842. Here, the roof was again reconsidered and the outer facade plastered.

In 1932 modernization and renovation works were carried out. The nave of the church was lengthened by demolishing the choir. The new choir was built in the form of a semicircular apse. A new sacristy was built on its northern side, which was previously located on the western side and was demolished. Furthermore, the entrance side of the church received a vestibule and a circular window. Further measures were the renovation of the altar, the roof structure and the restoration of the baroque organ. The leading architect for the expansion measures was Hans Ehl. The converted Sebastian Church was consecrated on January 13, 1933.

In German times only Polish services took place in this church, hence the name “Polish Church”. Nowadays the German church services are celebrated here, which take place every Sunday at 10 a.m.

In the interior there is a baroque organ that comes from the former Jesuit church of the city of Opole.
