Nursing rounds

from Wikipedia, the free encyclopedia

The care visit is an instrument within the framework of quality management for professionally performed care . It basically consists of a visit by two nurses to the patients or residents in their rooms of a care facility or, if the person lives privately, at their home. In a conversation, the need or wishes for nursing support should be updated. So it differs from the doctor's round in hospital operations . In contrast to a doctor's visit, it is not about diagnosing an illness, about daily changes in blood values, blood pressure or observing wound healing or body temperature. It is about the joint assessment of the progress the patients have made in dealing with their disease and the nursing support.

One nurse should be very familiar with the person being cared for and their care. The other nurse, who can be a manager, should have professional experience and be able to assess the plausibility of this information. The conversation with the patient or resident may reveal indications of necessary changes to the care planning or the overall care situation. Possibly. Consequences have it in counseling other persons involved in the supply, such as the general practitioner or family caregivers are proposed. Therefore, the participation of a senior nurse on this visit can be seen as support for day-to-day care. With the introduction of the DRGs and in the course of further health policy objectives, it is increasingly used in hospitals, among others.

definition

“The nursing round is a regular visit or conversation with the patient about the nursing process. It serves to jointly name the care problems and resources or the care diagnoses, the agreement of desired care goals, the type, amount and frequency of the necessary care measures and finally the review of the appropriateness, economic efficiency and effectiveness of care ”(Heering, 2004).

Contents of the conversation during the nursing round

In this conversation, the care problems and resources , the care diagnosis , the formulation of the care goals and measures, as well as the successes of previous care are briefly presented and the satisfaction of the person being cared for with the care is asked. Often there is the opportunity to make suggestions to the person being cared for on how to become active on their own.

In contrast to this, the term “ supervising nursing rounds” wants to place more emphasis on the control instrument in the context of quality assurance. It depends on the scope and effectiveness of the agreed maintenance measures.

From the nursing documentation are nursing anamnesis , care planning and ongoing maintenance reports zoom pull and assess their quality.

In addition to the care visit, individual authors also speak out in favor of a local visit and an organizational visit.

Difficulties in implementation

If the patient and their (carer) relative are prevented from attending.

It will not always be possible to find a common basis for communication with the patient. This may be due to the time within the usual daily rhythm or to the lack of sympathy on the part of the person being cared for. Then a shortening of the conversation may have to be accepted and the nursing round can only concentrate on those parts of the task that can be done without the person being cared for.

The time pressure in nursing makes it necessary to carefully plan with whom and how often the rounds are to be carried out. This can include the restriction on the connection with the discharge to another care facility, increased care expenditure in care level II or III z. B. mean after the home or in special cases of conflict. It would be desirable to have rounds at a fixed cycle for all persons being cared for. This would provide a broader basis for assessing the quality of care at the facility.

Follow-up and documentation

The main contents of the care visit are documented in the care report (report sheet of the care documentation ) or the evaluation column of the care planning or in a separate protocol. If necessary, a second appointment is made immediately, by which the responsible nurse has dealt with the issues in question.
