Prostate artery embolization

The prostate arterial embolization (PAE), is a treatment technique of benign prostatic hyperplasia (BPH), which promises less side effects. It is based on a method of interventional radiology that has been used for over two decades, embolization with particles, mostly so-called microspheres. The treatment is very similar to uterine fibroid embolization , which has been used since 1995, is based on the same principle and works in a similar way.

PAE is a minimally invasive catheter-assisted treatment that takes place through the arteries. The capillaries and precapillaries of the prostate are super-selectively closed with the embolizate, which leads to the shrinking of the enlarged prostate tissue. First prospective studies showed a similar effectiveness of PAE compared to TURP, with PAE showing significantly fewer side effects, especially with regard to incontinence and retrograde ejaculation, with a somewhat lower improvement in urine flow. However, larger studies and longer long-term courses are needed to confirm these results; Likewise, the assumed advantage of bilateral versus unilateral PAE has not yet been conclusively clarified.
