APRV is the only ventilation mode that primarily supports expiration and not inspiration . A preselected airway pressure is generated at which the patient can breathe spontaneously.

history

APRV ventilation was originally described by Stock and Downs in 1987 as CPAP ventilation with an intermittent release phase. APRV starts at a pressure higher than the base pressure and follows with deflation to achieve tidal ventilation. In principle, APRV ventilation is a time-dependent change between two levels of positive airway pressure, with the main time being at the high level and a short exhalation to facilitate ventilation.

  • The lower pressure level depends on the severity of the oxygenation disorder.
  • Opening time: 0.5–3 s.
