A cash request is the request of a patient to his health insurance company for approval of a health insurance funded psychotherapy in Germany. It must meet the requirements of the psychotherapy guidelines . The health insurance application must be accompanied by a report to the expert , which is prepared personally by the treating psychotherapist . The application and the report are submitted by the responsible health insurance company to a qualified external expert who will decide whether the psychotherapy applied for complies with the psychotherapy guidelines of the Federal Joint Committee . The guidelines call for a “comprehensive theoretical system of disease development” and proof of the therapeutic effectiveness of the specific treatment methods. The aim of the application and expert assessment process is to prevent unsuitable, i.e. non-indicated or prognostically unfavorable treatment attempts. The guideline and the expert procedure are not only intended to protect the solidarity community from burdens that can no longer be financed, but also to protect therapists and patients from harmful emotional entanglements and unreflected alliances. The application and appraisal process can - correctly understood - also be a contribution to the psycho-hygiene of the therapists.

Health insurance application for psychotherapy based on depth psychology (TP)

The psychodynamic psychotherapy includes etiological sound therapies with which the unconscious psychodynamics currently effective neurotic conflicts in compliance with transmission , countertransference be treated and resistance. A concentration of the therapeutic process is aimed at by limiting the therapy goal, by a predominantly conflict-centered approach and by limiting regressive processes. The guidelines allow for special forms: short therapy, focal therapy , dynamic psychotherapy and low-frequency therapy in a long-term, stable therapeutic relationship. Gestalt therapy , logotherapy , psychodrama , respiratory feedback, transaction analysis and other procedures for which the Federal Committee of Doctors and Health Insurance Funds cannot determine that the requirements of the guidelines are met may not be used at the expense of the statutory health insurance companies .

Health insurance application for behavior therapy (VT)

In addition to analytical and depth psychology-based psychotherapy, behavior therapy has been recognized as the third guideline procedure since 1987. The expert checks the health insurance company's obligation to provide benefits based on the following criteria:

  • Is there a disorder with a disease value (based on ICD 10 )?
  • Is there an appropriate treatment concept (based on a scientifically founded disorder model and the corresponding intervention methods)?
  • Is the treatment economical (is it possible to realistically improve the findings after a maximum of 80 sessions)?

Practical process

(Example long-term therapy)

  • The patient comes with her electronic health card and a transfer.
  • The required information is explored in a maximum of four probatory sessions plus an anamnesis survey .
  • Due to the complexity of the disorder, a long-term therapy application is made.
  • The patient applies for psychotherapy using the PTV 1 application form.
  • The therapist fills out a PTV 2 application form and attaches the report to the expert .
  • The fund receives both applications.
  • The expert receives the following in a closed envelope via the cash register: report, PTV2, clinical reports and, in the case of psychological psychotherapists or child and adolescent psychotherapists, the consulting report of a doctor (patient data as a code)
  • If the above criteria are met, the expert recommends that the costs of the therapy be covered.
  • The appraisal procedure will be abolished on December 31, 2022.
  • The expert procedure for group therapies has been abolished since November 23, 2019.
