The health insurance number is used to identify the insured person with a health insurance company . The health insurance number is required so that service providers, e.g. B. Doctors or dentists can use the health insurance card to settle their services with the responsible health insurance company via the statutory health insurance associations.

Since the introduction of the electronic health card in 2012, the first ten digits of the health insurance number as well as the pension insurance number have been assigned once and remain the same for life.

The legal basis for the introduction of the uniform health insurance number is § 290 SGB V. This also regulates which body assigns the health insurance numbers.

The health insurance number has 20 to 30 digits. It consists of a 10-digit unchangeable part, which is legibly printed on the electronic health card, as well as a changeable part, which shows the insurance company in the form of the institution code of the health insurance company (9 characters) and, if applicable, data on the main insured person (10 characters), as well as a check digit (1 digit) contains.

The first digit of the health insurance number is a randomly assigned capital letter (no umlaut!), Followed by eight random numbers, the tenth digit is a check digit that is weighted using the modulo 10 method described below, a modified Luhn algorithm 1-2-1-2-1-2-1-2-1-2 is calculated: The letter is replaced by a two-digit number according to its position in the alphabet (A = 01, B = 02, ..., Z = 26) , the resulting ten digits, together with the eight random numbers, are now multiplied by 1 and 2 alternately from left to right, followed by a cross-sum of the individual products with subsequent summation of the ten cross-sums. The check digit is the last digit of this sum (29 results in 9, 53 results in 3 etc.), which is calculated in the EDP using the modulo function and the divisor 10 - hence the name of the procedure.

Pension insurance number and health insurance number

For reasons of data protection, the pension insurance number may not be used at the same time as a health insurance number because of the personal data it contains. The statutory health insurances have set up the “Trust Center Health Insurance Number” (VST) at the ITSG . It creates a health insurance number for every citizen based on the pension insurance number. Citizens who have not yet had a pension insurance number will receive one for this purpose.

The health insurance companies send the pension insurance number (RVNR) and a file number to the VST. An auxiliary number (KVNR auxiliary number) is calculated from the RVNR using a hash function RIPEMD-160 . The procedure may only be used within a trust agency in order to prevent third parties from being able to determine the KVNR from a known RVNR. Therefore the check digit is additionally encrypted with a secret key. The KVNR auxiliary number is created in a secure environment. However, the use of a 20-digit number is not practical from the point of view of the consumer, as in some procedures the health insurance number has to be manually transferred to a template and errors can easily be made. Therefore, every 20-digit KVNR auxiliary number should be randomly assigned a ten-digit KVNR. Only the KVNR, the KVNR auxiliary number, the file number of the health insurance company and the institution code of the requesting health insurance company are stored in the VST database.

Before 2012, the health insurance number was not the same for life, but was assigned by the health insurance companies according to their own internal systematics. In the event of a change of insurance company or a change in the insurance status, a new health insurance number may have been assigned. As a rule, the numbers were 6-12 digit sequences. With some health insurance companies such as B. the TK was coded in the insurance number, the date of birth and a serial number.
