It is estimated that four million people are affected by cataracts in the People's Republic of China - particularly in rural areas, where agricultural workers are exposed to high levels of sunlight and poor hygienic conditions. Treatment for cataracts is inexpensive, but it is difficult for residents of remote areas to reach hospitals that can provide such treatment. The Chinese rail network, however, extends to all provinces of the People's Republic of China. Relocating medical facilities to a train makes it possible to treat the eye disease even where there is no in-patient infrastructure. The model for the Chinese Lifeline Express is the Jeevan Rekha Express from India , which has been in use since 1991.

In 1997 the first set of Lifeline Express was one for the purpose by the Hong Kong politician Nellie Fong founded Foundation commissioned. The Lifeline Express is one of around 70 projects in which the Chinese government grants tax advantages for donors. With the money received in this way, further trains were set up in 1999, 2002 and 2009, so that there are four Lifeline Express trains available today, including double-decker cars .

A Lifeline Express set is stationed at a previously announced location for several months and enables the population to be treated free of charge during this time. To this end, each train is equipped with two operating theaters , beds for recently operated patients, a conference room and living quarters for the doctors working on the Lifeline Express . In the first 15 years, 130,000 people were treated in this way.
