In the middle of pain or Intermenstrualschmerz is one for mid-cycle (about 14 days before the next menstruation) occurring pain during ovulation (ovulation), the only part of the women - and then only irregularly - feels. It can occur one to two days before ovulation or as a result of the follicle rupture . The pain itself is described very differently: short or long-lasting, stabbing or pulling, from a slight pinching to severe cramps, but mostly localized on one side in the lower abdomen.

The cause of pain varies from person to person. Peritoneal irritation due to the bleeding accompanying the rupture of the follicle, or pain due to the enlarged, ripe follicle itself. Middle pain is not suitable as an indicator in natural family planning . A lack of middle pain should not automatically be interpreted as a sign of a lack of ovulation.

The German term "Mittelschmerz" is an internationalism and is used as a medical term beyond the borders of the German-speaking area.

Differential diagnosis

In the differential diagnosis of right-sided lower abdominal pain, other causes can also be considered, such as appendicitis , adnexitis , Crohn's disease , ulcerative colitis , tubaria , nephrolithiasis , cholecystolithiasis , gastroenteritis , food intoxication . For left-sided pain, diverticulitis should also be considered (more likely in older patients).
