The building was built near the former Tsar's palace in Sofia (today the National Art Gallery ) based on a design by the Viennese architects Hermann Helmer and Ferdinand Fellner the Younger . The building was completed in late 1906 and the first performance took place on January 3, 1907.

In 1923 the theater was badly damaged by fire during a performance. The following renovation with expansion was carried out by Kiril Tschoparow according to the plans of the German architect Martin Dülfer by 1929. During the bombing of Sofia in 1943/1944, the building was again badly damaged and repaired in 1945. The last major renovation took place in 1971–1975 when a special chamber music hall was built.

In 2007 the facades, the roof and the decorative elements were renovated and their gilding renewed.
