Care problem

A care problem is a restriction of the independence of a person in need of care in one or more areas of life (see model of supportive process care ). If the person concerned cannot compensate for this, nursing intervention is necessary.

Compensation can be provided through advice, guidance, support or partial or full takeover. The maintenance measures can also be combined. In the associated care planning , only care problems are taken into account, medical and general human problems are not taken into account. The nursing problems represent the second step in nursing planning. In some countries this step is covered by a nursing diagnosis .

Classification of care problems

  • Current Nursing Problems: Problems that require immediate intervention. They can be determined through patient observation, statements or objective measurements.
  • Potential Nursing Problems: Risk factors that, based on the nurse's experience and expertise, can become a nursing problem without any action
  • Hidden / Suspected Nursing Problems: Problems caused by observed behavior.
  • General care problems: Problems that have general validity and are assessed through care knowledge and care standards.
  • Individual care problems: Problems that arise individually from a life situation or illness in a patient.
