Care diary

A care diary is used to record home care services that a private carer provides for a person in need of care .

As a rule, people in need of care apply for long- term care insurance benefits from their care insurance . This forwards the application to the Medical Service of the Health Insurance (MDK), which determines the degree of independence of the person in need of care in a care report during a registered home visit to the person in need of care . Based on the report, the long-term care insurance company determines the appropriate level of care.

In practice, it happens that the information given by those in need of care or caregivers to the MDK about necessary activities does not correspond to the actual circumstances. Often certain required care services are not specified out of shame or simply out of forgetfulness, which in the worst case can have a negative effect on the determined need for help and thus on the determination of the level of care. Due to the time pressure of the MDK experts, it often happens that care-relevant issues that are important for the classification are not even addressed during the assessment.

By keeping a care diary this should be counteracted, i.e. H. This is to guarantee the complete recording of all necessary help. In practice, this is handled in such a way that every person involved in care and support is entered in the care diary for a longer period of time, e.g. B. two weeks before the visit of the MDK expert enters the degree of independence of the person in need of care. The basis for recording self-employment with a care diary are 64 questions that were defined in the guidelines of the National Association of Statutory Health Insurance Funds for determining the need for care. The health insurance's medical service answers the same 64 questions in order to determine the degree of need for care and ultimately the appropriate degree of care. The completed care diary is presented to the expert during the home visit, who usually takes the entries into account when making his findings.

A care diary is also important in order to have documentation in the event of an objection to a decision from the care insurance company that proves the actual care expenditure.

Keeping a care diary is voluntary. If no diary is submitted, this does not release the MDK reviewers from asking for information from those in need of care and the caregivers.

Most care insurance companies can request forms for a care diary by telephone or online. With the introduction of the second Act to Strengthen Care , the care diaries that were valid until the end of 2016 and in which care minutes were recorded are no longer applicable. Many care funds have not yet updated their care diaries.

In the past, nursing diary forms were mostly sorted according to the basic care activities listed in SGB ​​XI . Nursing aids who do not come up daily (washing hair) or who often come up daily (remind them to drink) do not fit into such forms. There is also hardly any space in such columns and rows for assistance services that are only to be provided depending on the form of the day.

With the introduction of the New Assessment Assessment (NBA), the assessment criteria became significantly more extensive. The degree of independence of a person in need of care is determined on the basis of 64 questions . A care diary must answer the same questions in order to offer added value as documentation, which means a considerable expenditure of time, especially for laypeople.
