The idea of the SMC stems from the deliberations of the pharmacists who have identified that in the regular access to electronic prescriptions in a pharmacy, exclusive access only by the pharmacist is insufficient. The pharmacy staff must also be able to access the prescription. For this, either each employee would have to be provided with a personal card, which is associated with high costs. Alternatively, this authorization is given for a health care facility (e.g. the pharmacy), for which the owner is responsible, who in turn can give his employees access to it. A technical description of the SMC is contained in the specification of the health professional card, which u. a. by pharmacists, doctors, dentists and others.

According to the previous specifications of gematik GmbH, the presence of the SMC is always a prerequisite for access to all data of the telematics infrastructure.

The National Association of Statutory Health Insurance Physicians (for doctors, including private doctors), Werbe- und Vertriebsgesellschaft Deutscher Apotheker mbH (for pharmacists) and the Federal Association of Statutory Health Insurance Dentists (for dentists) will be responsible for issuing the SMC, which must ensure during the issuing process that only authorized persons can receive an SMC.
