Serodiscordance

A sero-discordant relationship is a sexual relationship in which one partner is infected with HIV and the other is not. This is in contrast to seroconcordant relationships in which both partners have the same HIV status, i.e. both partners are either HIV positive or HIV negative. Serodiscordance contributes to the spread of HIV / AIDS, especially in sub-Saharan countries like Lesotho . Serodiscordance results from an unequal serostatus, for example between two partners or twins , one of whom is HIV-positive and the other HIV-negative.

Daily pre-exposure prophylaxis (PrEP) with TDF ( tenofovir disoproxil fumarate ) or TDF-FTC (with emtricitabine ) reduces the risk of HIV transmission in sero-discordant couples.

In couples where the man is infected with HIV but the woman is not, the use of sperm preparation techniques in conjunction with insemination or in vitro fertilization (IVF) with intracytoplasmic sperm injection (ICSI) has been shown to be very effective in order to the seroconversion of uninfected women and their fetuses to avoid. The Sperm washing can be used to reduce the risk of HIV transmission in HIV-positive men to lower as the infection takes place rather by the seminal fluid than the sperm itself. An Italian study from 2005 of 567 treated serodiscordant couples found no horizontal (for the woman) or vertical (for the child) HIV seroconversion.
