Gear phenomenon

The gear wheel phenomenon is a clinical sign that can occur in various extrapyramidal diseases, especially Parkinson's syndromes . It occurs as part of an increased muscle tone ( rigidity ). This results in a jerky yielding of the muscle tone with passive movement of an extremity . The extremity can only be moved in a choppy (saccade) manner against increased resistance, as if it were being pulled over a gear .

In clinical practice, the presence of a gear phenomenon is examined primarily on the wrist and elbow joint .
