Arthur Ernest Guedel

Arthur Ernest Guedel (born June 13, 1883 in Cambridge City , Indiana , † 1956 in Los Angeles ) was an American anesthetist . His name became known through the development of the stages of anesthesia named after him. He conducted extensive studies on the uptake and distribution of volatile narcotics , as well as the differentiation of the various stages of anesthesia . He also developed an oropharyngeal tube , the Guedel tube .

Guedel described in an article in 1920 (and in a book in 1937) stages of the depth of anesthesia in ether anesthesia with diethyl ether. On the basis of reflexes , movements, pupil size , breathing , pulse strength and consciousness status of the patient , he differentiated four stages of ether anesthesia: sleep stage, excitation stage, tolerance stage (with four sub-stages) and asphyctic stage. This scheme lost its importance in the second half of the 20th century with the development of modern anesthesia. With today's use of drug combinations ( balanced anesthesia ), among other things, the administration of opioids , which make the assessment of the pupillary changes impossible, and muscle relaxants , which suppress the patient's defensive movements. However, during induction of anesthesia by means of inhaled anesthetics , which is still practiced in children today, an excitation stage occurs.
