The starting point for the construction of the church is the veneration of a statue of the Virgin Mary found in this place in 1635 by a peasant girl. A variety of miracles were quickly assigned to the statue. After previous buildings as a chapel, after the great earthquake of May 4, 1910, today's basilica was built on this site in a unique mixture of colonial architecture and Byzantine style . The building, designed by the architect Lluis Llach Llagostera, was built between 1912 and 1926 and was of significant size in Costa Rica for the time. In the church you can see the shrine with the small statue of the Virgin Mary. The shrine was received on July 10, 1935 by Pope Pius XI. the title of a minor basilica awarded.

The statue of the Virgin Mary, La Negrita , a Black Madonna , is in the basilica from September to August 2 and is kept in the Cathedral of Costa Rica for a month in between. On August 2nd, the basilica is the destination of a great pilgrimage of believers from all over the country, many of whom walk the famous 22 kilometers to the basilica during the Romería . Although many people start the pilgrimage from locations across Costa Rica, some choose to crawl the 22 kilometers on their hands and knees as a show of piety.
