Credé prophylaxis

from Wikipedia, the free encyclopedia

The Crede prophylaxis , even Credésche eye prophylaxis , is against gonococcal preventive treatment of neonates with eye drops of a one percent solution of silver nitrate . This is painful and irritating to the conjunctiva ( conjunctiva ) and cornea , which is why topical antibiotics ( erythromycin , gentamicin ) or 2.5% polyvidone-iodine solution are used as alternatives today .

history

The Leipzig physician Samuel Theodor Quellmalz (1696–1758) described in the 18th century a "purulent vaginal flow of the mother (or the original gonorrhea of the father)" as the cause of the "suppuration of the eyes in newborns". The introduction of Crede prophylaxis in 1881 by the German gynecologist Carl Siegmund Franz Crede (1819 to 1892), the incidence rate (could incidence ) at potentially leading to blindness Gonoblennorrhoe be greatly reduced (a purulent conjunctivitis in newborns). The inflammation is transmitted from the mother at birth if she is infected with gonorrhea .

In Germany, this prophylaxis was required by law as part of the U1 preventive examination until 1986 , after which it was only recommended. Because of the undesirable side effects caused by the silver nitrate solution, such as burning eyes and reddening, but above all because of the extremely rare occurrence of gonoblennorrhea, this form of prophylaxis has been used less and less in recent years and has been replaced by more tolerable alternatives, in some cases even left out. Even today, ophthalmologists still require this measure to be carried out because of its high level of effectiveness, to which the parents must give their consent.
