Epicrise
An epicrisis ( Greek ἐπίκρισις from ἐπί "after" and κρίσις "assessment") or final consideration describes in medicine and psychotherapy a differentiated assessment of the course of the illness after the illness has ended.
It is a special, summarizing, critical review and interpretation of the medical history and the therapy initiated , usually addressed to a doctor providing further treatment . It is often part of medical documents ( referral , doctor's letter , discharge letter , etc.). An epicrisis should contain the most important information on the anamnesis carried out (prehistory), diagnostics (findings), the course, finally established diagnoses and possible differential diagnoses, the recommended medication , remission and possibly the prognosis .
