Hendrich fall risk model

The Hendrich fall risk model (HSM, also Hendrich scale ) is a care assessment instrument developed by US nursing scientists , in particular Ann Hendrich, for assessing the risk of a person in need of care or patient falling . As part of the nursing history , six so-called items ( factors ) are recorded and assessed, which allow an assessment of the risk of falling: history of falls, depression , confusion / disorientation , changed elimination , drowsiness / dizziness and inappropriate mobility / weakness. In accordance with the risk assessment, fall prevention is included in the care plan and adjusted if the condition changes. Other ways of assessing the risk of falls are the Morse scale and STRATIFY , which specifically deal with fall assessment ; other instruments such as the long-term care dependency scale record further long-term care risks .
