A nameless young woman in forensic medicine - nothing unusual for Maura Isles. But when the pathologist opens the body bag, she finds a dummy in it. Maura responds immediately and calls the ambulance. But no sooner has the young woman regained consciousness in the hospital than she kills a security guard and takes Maura with an accomplice and, after she can save herself, other patients as hostages - including Detective Jane Rizzoli, who is about to give birth. For a long time it remains unclear what the hostage-taker is aiming for with her brutal action. Only when Maura and Jane's husband, FBI agent Gabriel Dean, manage to uncover the secret of their origins, things start to move. The hostage-takers apparently have a message that they want to convey through the press and radio. But before that happens, the hospital is stormed by the police. Both hostage-takers are shot under dubious circumstances. "Mila knows," the young woman can whisper to Rizzoli before she dies. Immediately after her delivery, the police officer, together with her husband Gabriel Dean from the FBI and her team partner Barry Frost, set out to search for the mysterious Mila. Jane Rizzoli realizes that there is more to the act than an ordinary hostage-taking. Because she comes across a criminal organization that reaches up to the highest levels of government.
