Sokolow-Lyon index

The Sokolow-Lyon-Index (Sokolow-Index for short) is a diagnostic sign in the electrocardiogram for the enlargement ( hypertrophy ) of the heart muscle . It is determined on the basis of the height of the rashes in the chest wall leads according to Wilson , and therefore requires a 12-lead ECG. The sign is relatively insecure, but can be used as a hint.

Left ventricular hypertrophy

The index for the thickening of the left heart wall is formed from the height (in mV ) of

  • S in the derivative V 1 plus
  • R in the derivative V 5 or V 6 .

The higher R is counted. If the sum is greater than 3.5 mV, this indicates hypertrophy of the left ventricle. Due to its diagnostic properties, the Sokolow-Lyon index cannot be used to rule out hypertrophy in hypertensive patients.

Right ventricular hypertrophy

A total> 1.05 mV of R in V 1 and S in V 5 or V 6 suggests a thickening of the right heart wall . This is also known as "Sokolow II".

Other hypertrophy indices

A positive Lewis index can support the suspicion of left ventricular hypertrophy . The Whitebock Index supports the suspicion of right heart hypertrophy. A better sign of left ventricular hypertrophy is the amplitude of R in lead aVL.
