The St. Sebastian Chapel is located at Broicher Str. 180 in the Broich district of Rheindahlen , the city of Mönchengladbach ( North Rhine-Westphalia ). It was built in 1759. The chapel was entered under No. A 045 on June 2, 1987 in the monuments list of the city of Mönchengladbach . The nearby bus stop for line 026 "Broich Kapellchen" is named after the chapel.

The chapel “Zum Heiligen Sebastian” is fenced in on a larger chapel plot and free on all sides. It belongs to the parish of St. Rochus and dates from 1759. The framework is made with white plastered partitions. The chapel has a hipped tiled roof (place, ridge and ridges made of slate) with ridge turrets and bells. In a lintel is the following inscription: PLEASE SO YOU WILL RECOMMEND IOH 16 V 2 17 V 4. The chapel is worth protecting as a monument due to its age, design and folklore reasons. The chapel had to be relocated in 1954 due to the widening of Broicher Straße. The last repair work on the chapel took place in 2000.
