In addition to the flat-rate system ( DRG system ), defined services, mostly expensive drugs , medical products or complex treatments, are additionally paid for in hospital billing in Germany .

In the hospital, services are remunerated on a flat-rate basis with flat-rate case fees. However, some expensive services rarely occur and are not tied to individual service categories. A fair remuneration through flat rates is difficult here. For example, patients with inherited haemophilia require special preparations for surgery or bleeding that often significantly exceed the cost of the actual treatment. In order to remunerate such services, additional fees are provided for in the German flat-rate system.

Services for which additional fees can be billed are agreed each year by the representatives of the cost bearers and the hospitals in the case flat rate catalog. A distinction is made between priced additional fees, recognizable by the designation ZE and a numbering, and unpriced additional fees, recognizable by the ZE , the current year and a number. Priced additional fees are calculated by the Institute for the Hospital Remuneration System (InEK). The remuneration of unpriced additional fees is determined on a hospital-by-hospital basis in the budget negotiations in the hospital.
