A field of participants that had remained unchanged compared to the Monaco Grand Prix three weeks earlier entered the fifth round of the World Championship in Detroit.

With Nigel Mansell , Ayrton Senna and Nelson Piquet, three drivers equipped with Honda engines once again qualified for the first three places on the grid. Thierry Boutsen followed with a lap time that was more than a second slower than Piquet in fourth place, ahead of the reigning world champion Alain Prost as well as Eddie Cheever and Michele Alboreto .

While Nigel Mansell was converting his pole position into a lead, Stefan Johansson damaged his front wing in a slight collision with Derek Warwick . In addition, Satoru Nakajima first collided with Ivan Capelli and shortly afterwards with Adrián Campos .
In the third round, Piquet could no longer defend his third place against Cheever. He damaged a tire in the course of the duel and as a result had to make an unscheduled pit stop that threw him back to 21st place. Eddie Cheever then dueled Teo Fabi for third place. When the Italian started to overtake on the seventh lap, the two collided. Fabi then had to give up the race in the pits, Cheever needed new tires and fell back to the bottom of the field. This put Alboreto in third place, followed by Boutsen and Prost.
By the 25th lap, Prost got past Boutsen and Alboreto into third place. During the regular pit stops, a wheel nut stuck on Mansell's car, causing him to lose around ten seconds. Senna took the lead. Meanwhile, Piquet had fought his way back to fourth place. On the 43rd lap he passed Prost.
Senna won confidently because Nigel Mansell had dropped to fifth place on lap 53, behind Nelson Piquet, Alain Prost and Gerhard Berger . Eddie Cheever crossed the finish line in sixth.
