A medical director is the representative of the leading doctors of a hospital and, according to the hospital (organization) laws, is part of the management of a hospital in addition to the leading nurse and the head of economic and administrative services.

The office of medical director can be exercised full-time or part-time, permanently or temporarily. Many small to medium-sized hospitals recruit the medical director on a voluntary basis from the ranks of senior physicians or chief physicians . Many larger hospitals or university clinics employ a full-time medical director who, on the side, no longer performs any actual medical work in the hospital, but instead concentrates exclusively on management.

There is no clear and uniform definition of tasks for a medical director. The different organizational structures of the individual hospitals define their tasks according to their own needs. In addition, the task of the medical director is not clearly defined in the respective hospital law in every federal state. While in North Rhine-Westphalia only the office is designated, the Saarland dedicates a separate paragraph to the medical director for the definition of tasks.

The medical director is often responsible for ensuring medical care, which is defined, for example, in the service regulations of the Augsburg Clinic as follows: "Supervision of the management of the nursing service in medical matters, supervision of the implementation of supervisory orders in the medical field and compliance legal obligations, ensuring hospital hygiene as well as planning and coordinating the medical admission service and the placement of patients in the hospital. ”This comprehensive role description makes the scope of the office of the medical director clear.
