DRG grouper

A DRG grouper is an economic-medical EDP program for calculating the respective flat rate per case based on the diagnosis related groups ( DRG ; German : diagnosis- related case groups ).

background

On January 1, 2003, a performance-oriented and flat-rate remuneration system based on the German Diagnosis Related Groups (G-DRG) was introduced in Germany to pay for hospital services . While it was still voluntary in 2003, it has been mandatory for all hospitals in Germany since 2004, with the exception of psychiatric departments and hospitals or departments that are explicitly excluded by law.

In order to ensure uniform encryption, a set of rules ( German coding guidelines or DKR) was created. From the diagnoses and procedures as well as the age, gender, weight information for newborns, the number of hours of mechanical ventilation, the length of stay and the type of discharge (e.g. relocated, deceased, normal discharge, etc.), the DRG is determined by a from the Institute for the hospital remuneration system (InEK) created and published algorithm (definition handbooks). This algorithm is revised every year and republished by the InEK.

Groupers were programmed in order to collect the flat rate per case with an economic expenditure of time. They work with the predefined algorithm and make data entry easier thanks to a user-friendly interface. Only current groupers who have been certified by InEK may be used (see also German Diagnosis Related Groups ).

The grouper needs the following raw data for its calculations:

  • Main diagnosis
  • Secondary diagnosis
  • Procedure date
  • gender
  • Age
  • Reason for admission
  • Reason for admission
  • Reason for dismissal
  • Length of stay
  • holidays
  • Pick-up weight
  • Status of the length of stay "one occupancy day"
  • Duration of mechanical ventilation
  • Department type (main department / document department)
