Thread method

The thread method is a minimally invasive otoplasty operation . The method is one of the special types of otoplasty operations . In contrast to other special forms, the suture method does not use additional techniques from traditional otopex surgery (see otopexy ). It belongs to the closed ear surgery, because the auricle is not cut open to lay the sutures. In the opinion of the author of the thread method, it is suitable for all protruding ears and, besides Mustardé's ear surgery, is the only method in which the cartilage is not weakened by scratches. The Merck suture method is the most commonly used method of minimally invasive ear surgery.

A method called the EarFold or Earfold method has recently been offered as a minimally invasive method. However, it does not meet the criteria of a minimally invasive otoplasty operation because, like traditional oturgery (see traditional oturgery ), incisions are made from which skin is lifted from the cartilage, albeit to a lesser extent.

The technique of the thread method as well as its results and possible risks has been described by Merck. According to him, he developed it in 1995 and used it on a large number of patients from 1996 onwards.

Surgical method

From 2 to 3 stab incisions on the back of the auricle, permanent, non-absorbable (non-dissolvable) sutures are placed around the cartilage of the antihelix using a special technique , tightened and knotted without the ear being cut open. When the threads are tightened, the auricle moves towards the head. The cartilage is left intact. The attached ear is held in its new position by the threads alone. During the operation, the patient can check and help determine the new position of his ears in a hand mirror. If the cartilage structure of the ear protrudes sufficiently far into the earlobe, Merck also uses the thread method to attach protruding earlobes.

In 1995 Fritsch had operated on 13 patients with this technique, but not limited to them, but combined this technique with the conventional method according to Furnas. In his later publications, Fritsch abandoned this method because he suspected that lasting results can only be achieved if the cartilage is additionally pricked, scratched or deeply cut in and sometimes completely through the cartilage on the front of the antihelical fold, which is based on conventional otoplasty surgery Stenström corresponds. In the opinion of Merck, supplementation with the Furnas and Stenström method is not necessary, which he demonstrated with several thousand operated ears.

Possible complications and risks
