machine

The aircraft involved in the accident was a Vickers 812 Viscount with the model serial number 353, which made its maiden flight on February 14, 1958, before it was delivered to Continental Airlines in May of the same year , where it was registered with the aircraft registration N240V . In August 1960, the machine was taken out of service by Continental Airlines and transferred to Tennessee Gas Transportation Corp. (Tenneco) , where it was initially in operation as the N601T and from March 1973 as the N601TL . Tenneco leased the machine to Alidair in October 1975 and to Cyprus Airways from March to May 1976 . The machine was then registered on the Alidair in Washington, DC and exported to the Republic of China on June 9, 1976 , where it was commissioned by Far Eastern Air Transport with its new aircraft registration number B-2076 in September 1976 . From October 1976 to January 1979 the airline leased the machine to Mandala Airlines and from March 1980 to Bouraq Indonesia Airlines, where the Viscount with the Indonesian registration number PK-IVS was in operation. The narrow-body aircraft was equipped with four turboprop engines of the type Rolls-Royce Dart 525 equipped.

Flight history

A domestic flight Banjarmasin to Jakarta was carried out with the machine. 31 passengers had started the flight. The six-person crew consisted of a master, a first officer, a flight engineer and three flight attendants.

the accident

The aircraft was approaching the Indonesian island of Java and was already approaching Jakarta when the crew declared an air emergency at 06:04 local time . The pilots reported an extreme flutter of the machine. Air traffic control asked the pilots of a Fokker F28 flying in the vicinity to catch up with the Viscount so that the Fokker pilots could see any damage to the Vickers. At 0628, the pilots of the F28 realized that there was a problem with the right elevator and that the machine was falling to the left. After the right elevator of the Viscount was torn down, control was lost. The machine hit headfirst with the aircraft nose pointing 25 degrees down. All 37 people on board were killed.

Cause of accident

Inadequate maintenance was found to be the cause of the accident. The centering pin of the right Flettner rudder on the elevator was broken. This was considered a wear part and was only designed for 12,000 operating hours, but has never been renewed. With an operating performance of 33,000 operating hours, the maintenance interval was exceeded by 21,000 operating hours.
