An Antonov An-148 aircraft accident occurred on March 5, 2011 when an Antonov An-148-100E belonging to the WASO aircraft assembly plant broke apart during demonstration tests near Garbusowo and fell to the ground. All 6 people on board the machine were killed in the accident. This was the first and only fatal incident up to Saratov Airlines Flight 703 with an Antonov An-148.

plane

The affected machine was a two-month-old Antonov An-148-100E , developed in Ukraine and assembled under license from the WASO assembly plant (Voronezhskoje akzionernoje samoljotostroitjelnoje obschtschestvo) in Voronezh . The machine had made its maiden flight on January 3, 2011 and was number 41-03 . The machine was operated with the test number 61708 and was to be delivered to the Air Force of Myanmar . The twin- engine passenger aircraft was equipped with two turbojet engines of the Ivchenko Progress D-436-148 type. Up to the time of the accident, the machine had only been used for test flights, with 31 take-offs and landings.

Occupants and purpose of the flight

The aircraft was manufactured for the Myanmar Air Force and took off on its maiden flight on January 3, 2011. After completion of the factory test program, a training program for the Myanmar flight crew in an aircraft was started. On March 5, the aircraft made its 32nd flight. There were four Russian pilots and test engineers on board, as well as two representatives of the Myanmar armed forces. As future pilots of the machine, the latter should be made familiar with it. The machine should be delivered during the course of the year.

Planned route

On the test flight, after take-off from WASO's works airport in Voronezh, the machine was initially supposed to fly in a south-westerly direction to Rogachevka . After its overflight, the machine should turn south and fly over the places Liski and Rossosch on its flight route , in order to then change course in the north-west to the place Gredjakino . Afterwards, the flight was again to be flown eastwards in the direction of Ostrogoschsk and from there back northwards to Voronezh.

the accident

During the test flight, the aircraft suddenly broke apart in the air around 10:40 a.m. on the section between Rossosh and Gredjakino near Garbuzewo, Belgorod Oblast and fell to the ground. All six inmates were killed. Parts of the machine, including a part of the horizontal stabilizer and elevator, went down two to three kilometers from the main wreckage. The crash site was about 137 kilometers south of the departure airport. The plane crashed on the outskirts less than a hundred meters from the nearest houses, broke apart and burned out.

root cause

The cause of the accident was a simulated emergency descent, in which the aircraft was accelerated to a flight speed that was 110 km / h above the maximum permissible flight speed for which the aircraft type was designed. This resulted in abnormal aerodynamic loads that exceeded the drag force of the airframe, creating low-frequency vibrations along all axes of the airframe, which ultimately led to structural failure and breakdown in the air. Inadequate coordination between the crew members during the initiated emergency descent, as well as deviations from recommended procedures and misleading displays of the flight instruments during a flight outside normal operating conditions were found to be contributing factors to the accident.
