Intensified rehabilitation aftercare

The intensified rehabilitation aftercare (short: IRENA ) is an aftercare concept of the German pension insurance that has been offered since 1999. The program can be used by those insured with the German Pension Insurance after inpatient or outpatient rehabilitation .

Following an inpatient or full-day outpatient service for medical rehabilitation , rehabilitation aftercare may be considered. This is intended to consolidate the rehabilitation success that has occurred. The rehabilitation patients should be able to stabilize and develop the compensation strategies and behavior changes that they learned in rehabilitation in everyday life through aftercare.

From 2016, a new framework concept for rehabilitation aftercare will apply in the statutory pension insurance. A transition period of three years from 2016 is planned for its implementation.

Basic principles of the "new" rehabilitation aftercare:

  • regardless of place of residence and pension insurance provider
  • close to home
  • needs-oriented

  • Intensified rehabilitation aftercare IRENA (multimodal)
  • Rehabilitation aftercare RENA (unimodal): Training-therapeutic rehab aftercare (T-RENA), rehabilitation aftercare for mental illnesses (Psy-RENA), rehabilitation aftercare for addiction disorders (addiction aftercare)
  • Rehabilitation sport and functional training according to Section 64 Paragraph 1 No. 3 and 4 SGB IX.

Tele-rehab aftercare has been available since 2017 .

Indication areas

  • the further improvement of still limited skills
  • the perpetuation of lifestyle changes and self-efficacy effects
  • the sustainable and verifiable implementation of what has been learned in everyday life
  • the promotion of personal and social skills

execution

The indication-specific therapies are carried out under expert guidance in group therapies, with or without therapy devices. For this purpose, the doctor at the rehabilitation facility issues a recommendation of the appropriate measures for the patient. Depending on the indication, the participant is entitled to a maximum number of therapies:

  • 24 appointments for diseases of the musculoskeletal system, diseases of the cardiovascular system and metabolic diseases
  • 26 appointments for mental disorders
  • 36 appointments for neurological diseases

Duration of therapy

The duration of therapy depends on the area of ​​indication.

Musculoskeletal system, cardiovascular system, metabolism, mental disorders

  • Start of the aftercare measure within 3 months after discharge from rehab
  • End of aftercare up to 6 months after discharge from rehab

Neurological diseases

  • Start of follow-up care within 3 months after discharge from rehab
  • End of aftercare up to 12 months after discharge from rehab

Limitations of the IRENA

  • Spatial availability: limited (e.g. in rural areas)
  • Temporal availability: limited (e.g. opening times of the centers)
  • After medical rehabilitation there can be waiting times of 4–12 weeks until aftercare begins
  • Therapy carried out in groups, no individualized individual therapy

Due to the restrictions mentioned above, tele-rehabilitation aftercare is a useful addition to the range of aftercare services provided by the pension insurance.
