Knee-heel attempt

The Knee Heel Attempt (KHV) is a step in the clinical nervous system exam that is used to review target lower extremity movements as part of the coordination tests.

The examinee moves the heel of the previous leg in an arched motion with his eyes closed onto the kneecap of the other leg. Sometimes the shin should be lowered with the heel, which may increase the sensitivity of the test.

The test is carried out lying down.

Results

  • Eumetry : target is hit.
  • Hypermetry : Movement goes beyond the goal
  • Intention tremor: increasing tremors as you approach your goal
  • Hypometry : goal is not (completely) achieved

Hypermetry and intentional tremor are indicative of a functional disorder of the cerebellum ( cerebellar ataxia ) or the spinal cord ( spinal ataxia ); hypometry is usually the result of paralysis of the moving leg.
