Lewis Sumner Syndrome

The Lewis Sumner syndrome , also MADSAM ( English : multifocal acquired demyelinating sensory and motor neuropathy "Multifocal acquired demyelinating sensory and motor neuropathy") is a neurological disease that belongs to the group of neuropathies . Neuropathies are diseases of the peripheral nervous system . In Lewis Sumner syndrome, as can be seen from the acronym “MADSAM”, both the motor and sensory nervous systems are affected. Among other things, this leads to paralysis and impaired sensitivity. The cause of the disease is not known for sure. It is considered to be immunologically caused and a special form of chronic inflammatory demyelinating polyneuropathy (CIDP).

The clinical picture is named after the authors of the first description, A. Lewis and AJ Sumner, from 1982.

Symptoms

The Lewis Sumner syndrome is characterized by the occurrence of asymmetrical paralysis (paresis) and sensory disturbances, caused by demyelinating nerve damage . In the affected patient, there is a disruption of the body nerves that occurs in many places, which leads to sensory and movement disorders, for example to a weakening of the sense of touch , painful abnormal sensations and muscle weakness .

diagnosis

The diagnosis is made through the history , neurological-clinical examination and electrophysiological examinations. Imaging ( MRI ) or nerve biopsy may also be necessary to distinguish it from other neurological diseases .

A differential diagnostic differentiation is necessary in particular from multifocal motor neuropathy and polyradiculitis (especially chronic inflammatory demyelinating polyneuropathy ), whereby MADSAM is viewed as an asymmetrical variant of polyradiculitis.

therapy

The therapeutic results achieved with antibodies (as intravenous immunoglobulins ) and glucocorticoids are quite good. The possible use of the latter in particular makes the differentiation from multifocal motor neuropathy so important.
