ICD-10 online (WHO version 2019)

As osteoarthritis (from gr. Ὦμος (OMOS) 'shoulder' and osteoarthritis ) a joint wear in is shoulder joint called. This type of osteoarthritis causes abrasion of cartilage in the head of the humerus and / or the shoulder socket.

causes

  • primary omarthrosis without any recognizable cause as age-related wear of the joint
  • secondary omarthrosis
    • in the case of previous bone fractures involving the shoulder joint
    • as a result of humerus head necrosis.

X-ray sign

Omarthrosis (above) compared to a healthy shoulder

The X-ray shows a deformation of the humerus head in the sense of spur-like extensions ( osteophytes ). By reducing the thickness of the articular cartilage, the visible joint space narrows. Upwards, towards the acromion , there is also often a narrowing as a sign of wear of the rotator cuff . But this is a pathological change in its own right that can also occur without omarthrosis.

consequences

As a result of osteoarthritis it can become too

  • Restricted movement in the joint
  • movement-dependent pain in the joint
  • inflammatory activation of the joint

come.

Differential diagnosis

Pain and restricted mobility of the shoulder also occur with calcification of the tendons (in the area of the rotator cuff, "calcified shoulder "), further alternatives can be found under frozen shoulder .

treatment

  • drug measures, e.g. B. hyaluronic acid
  • Pain management
  • physical measures
  • Cooling of the joint in the event of inflammatory activation
  • Irradiation of pain with a low dose of X-rays

Above all, it is important to maintain mobility. The immobility often results from an unfavorable posture: rounded back, hunched shoulders, an imbalance of the front and back muscles. There are a variety of exercises with light resistance that are suitable for straightening the body and usually also improving the symptoms. Often the external rotator cuff is not muscular enough, exercises for external rotation with a one-kilogram dumbbell, Theraband or similar devices help in the long term to improve the mobility of the shoulder and also to relieve pain.
