Perineurioma

The perineurioma is one of the benign tumors of the nerves . It consists almost entirely of neoplastically changed cells of the perineurium . In the normal state, these serve to combine the nerve fibers in bundles. In addition, perineuriomas of the soft tissue without any direct connection to nerves are observed. Both variants of perineurioma are extremely rare and make up less than 1% of all nerve tumors. According to the WHO classification of tumors of the central nervous system , they are mainly classified as grade I.

Clinically, intraneural perineuriomas appear as swellings of less than 10 cm with swelling and loss of function. they become symptomatic from muscular atrophy . Metastases are not known. Macroscopically, a segmental thickening of the nerve cord can be observed in biopsies . Histologically, there are concentric layers of neoplastic cells around the nerve fibers, which are reminiscent of an onion skin. Soft tissue perineuromas show up as 1.5 to 7 cm lumps. Histologically, they show a strand-like structure with spindle-shaped epineural cells, in some cases.

Perineuriomas can be removed surgically, but this is usually associated with the loss of the remaining intact nerve fibers, so that one usually waits after a diagnostic biopsy . There are no recurrences and metastases .
