From 1978 to 1980 he was a lecturer at the Institute for Ethnology in Uppsala and from 1981 to 1991 at the Institute for Ethnology and Anthropology at the University of Copenhagen. Aaby founded it in 1978 and has since headed the Bandim Health Project in Guinea-Bissau , West Africa , a research project on population-based health research. At its core is a health and population monitoring system that collects information on health, diseases, vaccinations, breastfeeding, etc. from more than 100,000 people in urban and rural areas, with an emphasis on women and children. The project is based on the collaboration between the Ministry of Health Guinea-Bissau and Danish scientists from the state immunological research institute Statens Serum Institut , the Syddansk Universitet and the University of Aarhus . It is a member of the INDEPTH network for health and population monitoring in Africa, Asia and Oceania.

In evaluating the data recorded in the Bandim Health Project , Aaby documented heterologous effects of vaccines. " Heterologous effects " or "off-target effects" are non-specific effects of vaccines that go beyond the specific protective effects against the target diseases. The effects depend on both the vaccine and the gender of the infant.
