Pharmaceutical care

The pharmaceutical care is to achieve the consistent perception of the responsibility of the pharmacist in drug therapy with the goal to provide therapeutic results, which are suitable for the quality of life of patients to be improved.

Pharmaceutical care is a complementary offer by the pharmacist , which supports the doctor's therapy in order to minimize drug-related problems.

aims

  • Optimization of drug therapy.
  • Recognizing and solving drug-related problems (e.g. with the help of the SOAP scheme = collecting subjective and objective information, analyzing and planning = subjective objective assessment plan)
  • The pharmacist assumes responsibility.
  • Assessment of the success of the application subjectively from the patient's point of view.
  • Cooperation with the other health care professionals .
  • Creating a care plan
  • Improving the patient's quality of life.
