A diaphragmatic relaxation is an extreme slackening of one half of the diaphragm with a permanent elevation of the diaphragm .
causes
A distinction is made between an innate and an acquired form.
innate form :
- Malformation of the phrenic nerve
acquired form :
- Birth trauma with unilateral or bilateral paresis of the phrenic nerve
- unilateral: intraoperative damage to the nerve, thoracic trauma , malignancy growth in the area of the nerve, intrathoracic (subphrenic) abscess
- bilateral: neuropathy of the phrenic nerve; Myopathy
Clinical manifestations
congenital form : a bilateral relaxation is usually not viable. In the case of unilateral relaxation, the infants are conspicuous by acute shortness of breath and heart failure.
Acquired form : shortness of breath, tachypnea, and recurrent pneumonia . About half of the patients are symptom-free.
Stomach volvulus can occur as a rare complication .
Diagnosis
Usually, the diagnosis is made based on the clinical picture of the disease in conjunction with a chest x-ray.
therapy
Surgical diaphragmatic tightening should be performed if breathing is clearly impaired.
