Testicular sperm extraction

The testicular sperm extraction (also testicular Spermatozoenextraktion , TESE ) is a method of treatment of reproductive medicine . During the TESE, sperm are taken from the man's testicular tissue, which can then be used for fertilization of the egg as part of a fertility treatment.

A TESE is carried out if the ejaculate does not contain any fertilizable sperm cells due to azoospermia (no sperm in the ejaculate), aspermia (no ejaculate), cryptozoospermia (very little sperm in the ejaculate) or necrozoospermia (no live sperm in the ejaculate). In addition, if there is pronounced teratozoospermia (too little or no normally formed sperm in the ejaculate) , a TESE can lead to better fertilization results.

Before the treatment, correctable hormone imbalances or other impairments should be ruled out, as only a few TESE treatments are possible and subsequent complete sterility of the man cannot be ruled out.

history

The procedure for obtaining sperm from a testicular biopsy was developed in 1993.

The engagement

During the procedure, which takes place under anesthesia, pieces of tissue are removed from one or both testicles and samples are examined to determine whether there are sperm cells in the removed tissue. The removed pieces of tissue are then frozen ( cryopreserved ) in order to be used for later fertility treatment.

During the procedure, the testicles are exposed on one or both sides via a small incision on the scrotum and the testicle capsule incised. Then the tissue pieces are removed and checked. The cuts are usually sewn with self-dissolving threads that should not be pulled.

The removed piece of testicular tissue contains both sperm and cells that produce sperm and sex hormones. Since the piece of tissue removed is very small, the risk of subsequent impairment is rather low. Due to the additional low operational risk, the procedure is usually carried out on an outpatient basis.

The subsequent healing time is usually given as 14 days.

Combined fertility treatments

Since the removed sperm are usually very limited in mobility and number, they are only used in combination with an ICSI , in which several egg cells are removed from the woman within a stimulated cycle and fertilized in the laboratory with direct injection of the sperm cell. A combination with an IVF or IUI is not recommended.
