The computer game was developed by Tango Gameworks and published by Bethesda Softworks on October 12, 2017. It was released for the Windows , PlayStation 4 and Xbox One platforms in English , German and many other languages.

Three years after the events at the Beacon Mental Hospital, Sebastian Castellanos is no longer with the Krimson City Police Department. His daughter Lily and her nanny died in a fire in the family home, his wife has disappeared and Castellanos, who is plagued by dark visions, is addicted to alcohol. His ex-colleague Juli Kidman, who still works for the MOBIUS organization, tracks him down in a bar . She reveals to him that Lily is still alive and that MOBIUS only faked her death to take care of the girl and use her as the core in the new STEM system. Union, an idyllic small town, is simulated in the middle of this system. However, since contact with Lily and the employees in the system was broken some time ago, Kidman asks the new hope-creating father on behalf of the mysterious administrator for help in finding the girl in Union.

Arriving in Union, Sebastian, lying in a bathtub under the watchful eyes of Kidman, quickly finds out that the place is populated by cruelly disfigured mutants and other dangerous creatures, all of them former "residents" and employees. In the course of his ride in hell, Sebastian meets the insane photographer Stefano Valentini, who has infiltrated the system and wants to use Lily for himself, the MOBIUS agents Hoffman, Sykes, O'Neal and Torres, the apparition Anima, the fanatical "priest" Father Theodore and finally his wife Myra, who also joined MOBIUS to save their daughter.

While the agents Hoffman, Sykes, O'Neal and Torres are more or less helpful to Sebastian and gradually die, it is up to Sebastian to face his demons and to bring down Stefano, the anima and Father Theodore. Finally, even Myra mutates into a gigantic creature, the matriarch , who, in love for her daughter, hatefully opposes the too often careless father. Myra regains her senses through her death and finally helps her husband by staying behind and sacrificing herself in order to destroy the system from within as a new core . This leads to the death of all employees in the research facility, including the administrator , as they were mentally connected to the STEM system by means of a chip in the neck . Kidman is the only one surviving, having removed her chip some time ago as part of her plan to destroy MOBIUS . Father and daughter finally separate from their wife after their escape from the nightmare, and the fate of the three characters is uncertain.

In a video sequence after the credits in the now abandoned MOBIUS facility , you can see that the STEM system is reactivated, which could imply that Myra still exists as the core within the system.

Gameplay

The game contains a single-player campaign in which the player controls ex-police officer Sebastian Castellanos from a third-person perspective . To survive, the player must run, sneak and fight through the game world.

The game world is built dynamically so that it adapts to the actions of the player. The player must find syringes and medicine packs to restore his life. The player can collect green gel , which can be found in numerous places in the game. Red gel can also be collected in a few places .

Unlike in the first part, it is possible to complete levels in different ways or not to attack opponents that are not relevant for progress. Often, by killing so-called underbosses , you get large quantities of green gel or, for example, urgently needed parts for the flame thrower. For the first time it is also possible to hide in the open in the middle of hedges from opponents who have absorbed the weather.

The newly introduced Communicator enables not only the view of area maps but also access to documents found and can also mark collectibles and so-called resonance points in the game world, which provide information about the fate of various former residents.

In the course of the game, the player receives a knife, a revolver, a pistol, a magnum, three different shotguns, a sniper rifle, an assault rifle, a flamethrower and the so-called torture crossbow . The crossbow fires various arrows, which can freeze, blind, shock or explode opponents or injure them with harpoons. In contrast to the predecessor, a wide variety of raw materials can be found in the game world, with which all ammunition and healing items can be produced on workbenches. It is also possible to use found weapon parts to improve the attributes of the weapons.

At some points in the game, Sebastian can get through a mirror to a retreat, which is modeled on the Krimson City police station. Here he can look at slides found in the world, use a workbench, complete two different shooting range competitions in the form of a mini-game, use keys to access the contents of several lockers or improve his own attributes with Sister Tatiana. Only in Sebastian's office and in a few sections that also contain mirrors is it possible for the player to save his progress and to be safe from the attacks of the opponents.

Production started back in March 2015, shortly after the team finished work on the first DLC of the predecessor. Shinji Mikami only took part as producer, whereas John Johanas was used as lead director . The game was officially launched at Bethesda's press conference at Electronic Entertainment Expo 2017 in July.
