Excellent patient care

The excellent patient care was a quality program in outpatient medical care in Bavaria from 2008 to 2011 and a brand of the Bavarian Association of Statutory Health Insurance Physicians (KVB).

History of origin

The trademark “Excellent Patient Care” was registered in November 2008 with the German Patent and Trademark Office . Under the umbrella of this brand, four regional quality measures from the areas of prenatal diagnostics , colonoscopy , outpatient operations and hygiene of flexible endoscopes were bundled. The decisive factor for the introduction of the brand was the entry into force of the Statutory Health Insurance Competition Enhancement Act ( Health Reform 2007 ). From the beginning of his term in office in 2001, the then chairman of the KVB, Axel Munte, pushed for the comprehensive and permanent safeguarding of optimal patient care. The main idea was to set incentives for outpatient doctors for proven quality above the nationwide requirements through quality-oriented remuneration. Until the end of 2008, KVB was in a contractual partnership with the health insurance companies in Bavaria and had all options open to them. With the law to strengthen competition in statutory health insurance (GKV-WSG), the possibilities to conclude regional contracts were significantly restricted from January 1, 2009. At the urging of the Bavarian state government, the inclusion of Section 136 (4) in the Social Code V was achieved in 2008 , which created a legal basis for the differentiation of medical remuneration according to defined quality criteria. On this basis, it was possible to continue further quality measures, which were bundled under the brand "Excellent patient care" from January 1, 2009.

After the change of the board of directors of the Association of Statutory Health Insurance Physicians in Bavaria in January 2011, it was decided to end the quality program "Excellent patient care" on December 31, 2011 due to the new strategic orientation. This primarily affects the use of the umbrella brand. The individual quality measures initially continue.

Goals and meaning

As part of the quality program "Excellent patient care", objectively verifiable aspects were picked out from various areas of medical practice and their quality was checked and ensured using defined criteria. For example, the hygiene of the instruments or the specialist knowledge of the doctor was checked. The quality standards to be verified were above the national requirements. Doctors participating in the program received a certificate and a seal of approval as proof of the above-average quality standards. In addition, they were listed in a special doctor search and in most cases received additional remuneration for participation. This principle of quality-oriented remuneration (Pay for Performance - P4P) was intended to promote quality, competition and transparency in outpatient medical care.

Content and scope

When it ended in December 2011, the quality program consisted of 18 different quality measures. Resident doctors in Bavaria from over 20 different specialist areas could take part in one or more measures if they met the relevant requirements for participation. Of the approx. 22,000 resident doctors in Bavaria, around 5,000 took part in the quality program.
