On July 31, 2010 he won the WBO world title in middleweight by technical knockout in the 5th round over the until then undefeated American Daniel Jacobs . Jacobs led with 39 to 37 votes on the judges' slips of paper until precipitation, but went to the ground 57 seconds after the start of the 5th round through a right straight, was counted and only slowly began to rise again at 5, whereupon referee Robert Byrd broke off the fight and declared Pirog the winner.

On March 26, 2011, he defended his title with a unanimous victory on points (115-112, 117-111, 115-111) against the Argentine Javier Francisco Maciel . On September 25, 2011 he denied another title defense against ex-European champion Gennady Martirosyan and won early in round 10. Another title defense and thus his last fight he denied on May 1, 2012 against the Japanese Nobuhiro Ishida ; Pirog won the fight unanimously on points.

In the summer of 2012, WBO signed Pirog to play his next title defense against Hassan N'Dam N'Jikam . Pirog decided to fight against WBA world champion Gennady Golovkin . At the end of August 2012, the WBO replaced Pirog in the list of world champions with N'Jikam on their website. Shortly afterwards, Pirog confirmed in an interview that the WBO world title had been stripped from him. However, the fight against Golowkin did not take place due to Pirog's back injury.
