First form

The upright rectangular badge is 140 mm high and 100 mm wide, made of iron and kept matt black. It shows an air force eagle on its front, holding the pommel of a sword pointing downwards with both fangs . The pommel of the sword shows an upside-down swastika. To the left of the sword is the depiction of the Wroclaw town hall and the banner waving underneath with the inscription: BRESLAU . To the right of the sword, the representation of Krakow Castle is symbolized, which is also surrounded by a banner with the inscription KRAKAU on the lower edge. Below these two images is the three-line inscription: FOR.GUTE.LEISTUNG / DER.KOMMANDIERDE.GENERAL / UND.BEFEHLSHABER.IM.LGK.VIII .

Second form

The second form of the badge differs slightly from its predecessor, with the inscription under the Wroclaw City Hall (left) now showing the inscription: SCHLESIEN and on the right instead of the inscription BRESLAU now GEN.GOUV. ( Generalgouvernement ) is to be read. The following three-line inscription is now changed: FOR.GUTE.LEISTUNG / THE.COMMANDING.GENERAL / AND COMMANDER .

The plaques of honor were presented to the recipient with an award certificate in A5 format.
