EndoCert

EndoCert is an initiative of the German Society for Orthopedics and Orthopedic Surgery (DGOOC) and represents the world's first system for the certification of medical facilities in the field of joint replacement . Around 400,000 joint replacement operations are performed in Germany every year. The quality of treatment is to be increased by specializing in facilities in the field of endoprosthetics.

Since 2012, medical facilities have been able to be certified as EndoProsthetic Center (EPZ) and as EndoProsthetic Center for Maximum Care (EPZmax) , provided that compliance with the requirements is proven in an audit .

Goals of EndoCert

The introduction of the EndoCert certification system is intended to ensure high quality patient care and high patient safety in endoprosthetics . The review of the structural and process quality is of central importance in the certification process. The facilities must meet numerous requirements and, in particular, demonstrate an interdisciplinary and continuously developed treatment path for patients and a high level of training for all professional groups involved in treatment. The review of the quality of results is possible in the long term through the mandatory participation in the Endoprosthesis Register Germany (EPRD) and in the short term through the recording and evaluation of specific parameters for the quality of care provided. The further recording of the quality of results according to patient satisfaction criteria ( PROM for “Patient Related Outcome Measures”) is currently being tested. The EndoCert system is constantly being further developed in the sense of a learning system.

In 2008 Wolfram Mittelmeier , Director at the Orthopedic University Clinic in Rostock, developed the basis for the EndoCert project. On his initiative, the German Society for Orthopedics and Orthopedic Surgery (DGOOC) set up a working group of experts that, based on the latest scientific findings, has developed criteria that increase patient safety and improve the quality of care in the Implantation of joint prostheses. In addition to representatives of the DGOOC, the German Society for Endoprothetics eV (AE), a section of the German Society for Orthopedics and Trauma Surgery (DGOU) , the Professional Association for Orthopedics and Trauma Surgery (BVOU) were also involved. Since October 19, 2012, after two pilot phases in which the developed criteria were checked, the procedure has been open to all institutions nationwide to participate. Since regular operations began, 343 facilities have been successfully certified. 138 other facilities are about to receive initial certification, so that in the medium term around 450 to 500 certified endoprosthetic centers will take part in the supply. (As of May 1, 2015). An actual improvement in patient safety and the quality of care through successful certification has so far not been scientifically proven.

Organization / structure

Organizationally, the certification system is carried out by three bodies. Overall responsibility for the system and its further development lies with the certification commission. The certificate issuing committee decides on the issue of the certificate. The processing and handling of the auditing process is carried out by the certification body. This clear separation of tasks ensures objective and independent implementation of the procedures and further development of the system. The procedure and the underlying provisions are laid down in rules of procedure.

Course of the procedure

Interested institutions have the opportunity to be certified as an endo-prosthetic center (EPZ) or endo-prosthetic center for maximum care (EPZmax) . These two centers differ in terms of the requirements for the structural and process quality as well as the number of cases to be provided.

The certification process consists of different phases. First of all, the institution making the application presents the implementation of the requirements in a self-assessment process by processing the questionnaire. This is followed by an initial assessment of the questionnaire by the certification body. Two experts carry out the certification on site. They check that the information given in the questionnaire matches the situation on site. On the basis of the recommendation by the auditing experts, the certificate issuing committee makes a decision on the issuing of the certificate.

The facilities are then audited by a technical expert as part of annual surveillance audits. A repeat audit takes place every three years, the form and scope of which corresponds to the initial certification.

If the requirements are not met, a certificate suspension or a certificate revocation can be pronounced.
