Erythema nodosum in tuberculosis

A erythema nodosum (also node Rose , dermatitis contusiformis or erythema contusiforme , plural: erythema nodosa) is an acute inflammation of the subcutaneous fat ( subcutis ), panniculitis called, with a contribution from the capillary walls and a nodule formation (granuloma). It occurs on both the extensor muscles of the lower legs , the knee and ankles , and less often on the arms or buttocks, and is very painful.

Characteristic of erythema nodosum is the formation of several blurred spots or nodules under the skin. These are slightly raised and very sensitive to pressure due to the underlying inflammation. The color varies from a reddish-purple to a yellowish-green hue, which is caused by the breakdown of hemoglobin . The erythema is usually accompanied by a general feeling of illness and fever; it itself is usually only a symptom of other diseases.

Erythema nodosum is generally regarded as an allergic overreaction ( type III ) of the skin that is associated with sarcoidosis , Löfgren's syndrome , various infections ( tuberculosis , streptococci , Yersinia , chlamydia , Campylobacter jejuni , cat scratch disease or toxoplasmosis ), inflammatory bowel disease (inflammatory bowel disease) , enteritis , ulcerative colitis ), Behcet's disease , drugs (eg. B. sulfonamides , penicillins , oral contraceptives ) as well as the rheumatic fever occurs.

Symptomatically, the disease can be treated with a non-steroidal anti-inflammatory drug ( indomethacin , acetylsalicylic acid ), or more recently with potassium iodide (doses of 300-1500 mg / day for a few days to 8 weeks) for Crohn's disease-associated, therapy-refractory erythema nodosum.

This dermatosis , formerly also known as Erythema induratum (Bazin) , Bazin disease and tuberculoid , was first described in 1861 by the French physician Pierre Antoine Ernest Bazin (1807-1878).
